INSURANCE
Elite Orthopedics/Sports Medicine participates in many major health insurance plans. Once we have accurate and complete health insurance data, we will file your insurance claim directly with the insurance company soon after your visit.
It is the patient’s responsibility to provide complete and accurate insurance information (including the name of the insurance company and policy number) to ensure proper filing. Usually all the information we need is found on your insurance card. Co-pays and co-insurance expenses are expected at the time of service. Patients are responsible for any balance not paid to EOSM by the insurance provider or if incomplete/incorrect information is submitted to EOSM resulting in an unpaid claim.
If you do not have health insurance, payment in full is expected at the time of service. Patients can expect to pay for the estimated cost up front and the remainder at the end of your visit. If you have overpaid, you will receive a credit or refund as desired.
