CLINIC APPOINTMENTS
For you convenience, appointments are available three days per week. Please call the clinic to schedule an appointment during one of the following times:
Clinic Appointments:
Monday: 1:00 p.m. – 5:00 p.m.
Wednesday: 8:30 a.m. – 12:00 p.m. AND 1:00 p.m. – 5:00 p.m.
Friday: 8:30 a.m. – 12:00 p.m.
Pre-Visit Instructions
Please bring the following to your first visit:
Drivers License
Insurance Card
Referral (if needed)
X-rays, MRIs, bone scans (if available)
Shorts, running shoes (may only be necessary for physical therapy)
You may also fill out the following forms prior to your visit, and either print them out or attach them to an email to be sent to: info@eliteorthopedics.net . We will have the forms ready for your signature when you arrive in the clinic:

Cancellation and Missed Appointment Policy
Our goal is to provide quality orthopedic and sports medicine care in a timely and professional manner. In order to accomplish this, we implemented a cancellation and missed appointment policy. This policy enables us to more efficiently utilize our resources to better serve our patients, provide you with the very best medical care, and accommodate all appointment requests.
As all medical services are provided by appointment only and this time is reserved for your exclusive use, we request 24-hours notice via a phone call to 281-997-3717 to cancel an appointment. You may not cancel appointments via email.
There is a fee of $25 for missed appointments and/or cancellations made less than 24 hours in advance.
Your cooperation and understanding in this matter are greatly appreciated.
Arriving Late for Physical Therapy:
If you arrive more than 15 minutes after your scheduled appointment for physical therapy, you must reschedule that appointment for another time. If you anticipate late arrival and call in ahead of time, your physical therapist, at his/her sole discretion, may still be able to accommodate you.
However, based upon the Physical Therapists patient schedule, you may still need to reschedule that appointment.
The Cancellation/Reschedule fee of $25 applies to all such appointments we are forced to reschedule as outlined above.
This policy is necessary so that the physical therapist can give you the proper attention and the best care possible.
